- Duly Accomplished Application form (1 original)
- Minor’s Birth Certificate SECPA(1 original, 2 photocopy)
- Colored passport size photo (3 original, taken within the last 6 months)
- Passport of minor’s travel companion.
- Notarize Affidavit of support and consent to travel and sponsor
- Income tax return of sponsoring person or proof of income/ bank and certificate of employment.
- Marriage Contract of minor’s parents SECPA
- Valid ID of Parents with specimen signature
- Notarized letter of consent of minor’s parents.
- Waiver signed by parents(this should be explicitly state releasing DSWD from any liabilities/responsibility in case of untoward incident)
Additional Requirements for Japan:
- Birth Certificate of Traveling Companion SECPA
- Invitation letter/Letter of Guarantee from sponsoring person
