Registration of Death

The death of a person shall be registered within the reglementary period of thirty (30) days at the Office of the City Civil Registrar.

CHECKLIST OF REQUIREMENTS WHERE TO SECURE

  1. Four (4) copies of the document to be registered (must be accomplished correctly and completely)
  2. Burial Permit
  3. Transfer Permit of Cadaver (if buried in other city/municipality)
