Court decrees concerning the status of a person shall be registered in the Civil Registrar’s Office where the court is functioning within ten (10) days after the decree/order has become final and executor.

  1. Original/certified photocopy of the court order
  2. Certificate of finality
  3. Certificate of court registration and authenticity issued by the concerned City/City Civil Registrar where the Court Order (CO) was issued
  4. Certificate of Live Birth (PSA Copy)
