Petition for Change of First Name (CFN)

Republic Act No. 9048 (RA 9048) authorizes the city or municipal registrar or the consul general to correct a clerical error or typographical error in an entry and/or change of first name or nickname in the civil register without need of a judicial decree.

CHECKLIST OF REQUIREMENTS WHERE TO SECURE

  1. Three (3) photocopies of PSA Birth Certificate
  2. Two (2) photocopies of each documents:
    1. LCR Copy of Birth Certificate
    2. Baptismal Certificate
    3. School Record
    4. NBI Clearance
    5. Police Clearance
    6. Clearance Certificate of no pending administrative or criminal case from employment (if employed)
    7. Affidavit of Unemployment (if not employed)
    8. Valid IDs
