CHECKLIST OF REQUIREMENTS WHERE TO SECURE

  1. For Medical Assistance
    1. Certificate of Indigency (1 original)
    2. Medical Certificate/Abstract or Hospital Bill (1 photocopy)
    3. Laboratory Request (1 photocopy, latest)
    4. Prescription of medications (1 photocopy)
    5. Government Issued I.D of claimant (1 photocopy)
    6. Letter addressed to the Mayor (1 original)

  2. For Burial Assistance
            - for indigent individuals/families whose members passed away and have difficulties in paying for burial expenses.
    1. Letter Addressed to the Congressman (1 original)
    2. Certificate of Indigency (1 original)
    3. Death Certificate (1 certified true copy)
    4. Government Issued ID of deceased claimant (1 photocopy)
    5. SSS/GSIS/Veterans ID of the deceased (1 photocopy)

  3. For Transportation Assistance
            - for indigent individuals who are stranded and whose families cannot afford to pay for their transportation expenses.
    1. Certificate of Indigency (1 original)
    2. Letter addressed to the Mayor (1 original)
    3. Doctor’s recommendation/referral to other institutions/hospitals/agencies and Medical Abstract (1 original per document)
