Where a marriage license is required, each of the contracting parties shall file separate sworn application for such license with the proper local civil registrar of the place where either or both of the contracting parties reside.

CHECKLIST OF REQUIREMENTS WHERE TO SECURE

  1. Duly accomplished Application Form (Contracting parties should both sign the application form)
  2. Certified true/xerox copy of birth certificate of applicants
  3. Certificate of No Marriage (CENOMAR)
  4. For Foreigner - Legal capacity to marry from the respective embassy in the Philippines
  5. Pre-Marriage Counseling Certificate & Family Planning Seminar
  6. Parental Consent if 18 yrs. old but below 20 yrs. old
  7. Parental Advice if applicant is 21 yrs. old but below 25 yrs. old
  8. At least one of the contracting parties must be a resident of the place where the local civil registry office is located
Additional:
  1. Certificate of Death (if widowed)
  2. Annotated Certificate of Marriage (if annulled)
  3. Court Finality (if annulled)
  4. Judicial Decree of Absolute divorce if divorce
