Petition for Correction of Clerical Error (CCE)

Republic Act No. 9048 (RA 9048) authorizes the city or municipal registrar or the consul general to correct a clerical error or typographical error in an entry and/or change of first name or nickname in the civil register without need of a judicial decree.

  1. Three (3) photocopies of PSA Birth Certificate
  2. Two (2) photocopies of LCR Copy of Birth Certificate
  3. At least two (2) or more of the following documents:
    1. Baptismal Certificate
    2. Marriage Contract (if married)
    3. Voter’s Affidavit / Certification
    4. Employment Record
    5. School Record
    6. GSIS/SSS Record
    7. Birth Certificate of Mother/Father
    8. Death Certificate of Mother/Father (if deceased)
    9. Marriage Contract of Parents
    10. Birth Certificate of Brother/Sister
